Case Report: Spontaneous Subarachnoid Haemorrhage in an Adult with Alport's Disease: A Case Report and Review of the Literature

Aneurysmal subarachnoid haemorrhage constitutes a common clinic-pathological entity carrying significant mortality and morbidity. We present a 34-year-old patient with Alport's disease-induced chronic renal failure, presenting with a gradual loss of consciousness following a thunderclap headache, after a dialysis session. The patient's head CT scan showed a severe subarachnoid haemorrhage, with increased thickness of blood in basal cisterns and diffuse intraventricular haemorrhage. Her CTA revealed a fusiform aneurysm of the left vertebral artery. Bilateral ventriculostomies were inserted for controlling her increased intracranial pressure and her hydrocephalus, and was managed in the ICU. She finally passed away after developing diffuse cerebral edema. The potential role of type IV collagen deficiency along with the renin-angiotensin complex dysfunction of Alport's disease in the pathogenesis of the cerebral aneurysm is examined in our current report. The pertinent literature is reviewed with emphasis on the association of kidney diseases with the formation of intracranial aneurysms.

Fahr syndrome is a rare anatomo-clinical entity. It combines bilateral and symmetrical calcification of Basal Ganglia (BG) and may be associated with dysparathyroidism. We report two cases of symptomatic Fahr syndrome in two Togolese. The first case concerns a 16-year-old boy with generalized tonic-clonic convulsions. The second case concerns a 39-year-old male adult with aphasia, behavioral disorders and dysphagia. Cerebral computed tomography showed bilateral and symmetrical intracerebral calcification of BG in both patients and a phospho-calcium balance revealed signs of hypoparathyroidism. Correction of phospho-calcium disorders had improved clinical manifestations.

Malignant glioma is one of the most aggressive and angiogenic human tumors and characterized by microvascular proliferations. Vasculogenic Mimicry (VM) is a newly described phenomenon associated with increased aggressiveness and poor overall survival in glioma. Aquaporin 1 (AQP1) is expressed by normal vascular endothelium and has been found involved in mediating cell motility and proliferation in other malignancies. We investigated the role of AQP1 in angiogenesis in glioma cell line. Our data show that AQP1 promotes VM formation in malignant glioma, it could be a potential target for anti-vascular therapy

The management of stroke is now recognized as a real medical emergency as well as myocardial infarct, because stroke units, fibrinolysis with rt-pa and mechanical thrombectomy in cerebral infarct, have dramatically changed prognosis of ischemic stroke with a decrease of mortality of 20 % and an increase of total recovery of 20 %.
This medical emergency enforces the care systems because it needs a speedy network for the patient, his family and the care professionals because it is necessary to start treatment as soon as possible to be efficacious.
